[Carpet] using IO*::one_file_per_group

Hi,

That would be a useful feature indeed.

In addition it would also be nice to be able to specify different 
out_every values to different variables. For example when we decompose 
psi4 into spherical harmonics we output a 2d grid array. I typically want 
to do that more often than 2d ouput of 3d grid functions.
