[Carpet] Checking proper nesting

I have just pushed a patch that makes Carpet check that the grid  
hierarchy is properly nested.  Proper nesting is a three-level  
relationship; it means (approximately) that grid points that are  
prolongated from level L-1 to level L are not themselves used to  
prolongate from level L to level L+1.  An additional parameter  
CarpetLib::proper_nesting_distance requires that there are  
additionally a few grid points separating these two regions.

If a grid structure is not properly nested, Carpet emits a level one  
warning during regridding.
